This report is designed to help recognize and reduce loss potential within the residence. This report should be completed by an alumnus immediately after a visit and walk thru of the house. This report is submitted directly to the Delta Nu Housing Board for review and discussions with the house manager. On a scale of 1 to 5 with 1 being unacceptable and 5 as great rate the items below. If you cannot answer the question, select the NA (Not Available) option.
